Please ask for information Online Access to your Medical Records The surgery is working towards allowing you to have online access to your medical records. Our plan is to have this service available, through SystmOnline, by 31st March 2015. You will then be able to access and print summary information from your medical record. This is currently limited to medications, allergies and adverse reactions. Booking Appointments PLEASE NOTE THAT AS OF THE 1ST JUNE 2014 IT WILL NO LONGER BE POSSIBLE TO BOOK APPOINTMENTS AT THE RECEPTION DESK BETWEEN 7.30 – 8.00AM. THE SURGERY WILL ONLY BE OPEN AT THESE TIMES FOR PATIENTS TO ATTEND PREBOOKED APPOINTMENTS. WE APPRECIATE YOUR COOPERATION IN THIS MATTER. THANK YOU.|
Shingles is caused by the reactivation of an infection of the skin resulting in clusters of painful blisters. Shingles is caused by the same virus that causes chickenpox. A new vaccination against shingles has been produced for all patients with dates of birth between 01.09.1934 - 31.08.1936 and 02.09.1943 - 01.09.1944. To enquire whether you are eligible for this vaccine or to book an appointment please contact the surgery on 01325 461128.SystmOnline is a new functionality that provides greater flexibility to our patients. It enables patients to order repeat prescriptions, prebook appointments, cancel appointments and complete questionnaires. To register for Systmonline please email us on firstname.lastname@example.org or telephone us on 01325 461128 and request a username and password. After receiving your login details it can take up to 4 hours to activate your account, so please refrain from using the system until then.

How your information is used and shared is controlled by law and strict rules are in place to protect your privacy. We aim to provide disabled patients with appropiate wheelchair access to our surgery. The surgery is on ground level and there is a disabled toilet to the rear of the building which can be easily accessed. Entrance to the building and to and from the waiting room is accessed via push pad automatic doors. If you require any assistance in accessing the building please telephone the practice from outside on your arrival. If you are hard of hearing, partially or non-sighted, please inform the receptionist when making your appointment or on your arrival that you may need extra assistance. If access proves difficult to any of our disabled patients we would be happy to consider any suggestions for improvement.
