If you are travelling abroad, you may require extra immunisations and injections. In order that we can organise to give you these, we need to know the full details of your travel plans. In order to do this, we have a travel form that needs to be completed ideally 4 - 6 weeks prior to travelling.
Itineraries that may be complex, ideally you should allow 2 months for all immunisations to be given. If you are visiting many destinations please give details of how long you will be staying in each country. Please attach an extra sheet if necessary.
Complicated itineries e.g. backpacking/multiple destinations. If left late, this may result in the surgery not being able to offer you an appointment. In this case you may need to go to a private Travel Clinic.
