Patients requiring sick notes do not always need an appointment with a GP for this as sometimes it can be issued without you having to be seen.

Please contact the surgery on 01325 461128 and explain to the receptionist that you are in need of a sick note and she will pass on the information to your GP.

If however, you have just been discharged from hospital we may not yet have received the information. Please bare with us during these times as the receptionist would then need to contact the department you were seen to request the information before a sick note can be issued.

If you have been on sick leave and are wanting to return to work, we no longer provide 'return to work notes'.

  2. MMR Outbreak!
    Due a recent MMR outbreak, all patients are encouraged to have this vaccination. If you are unsure whether you have had one or not, please contact us.
  3. Flu Vaccinations
    Our flu vaccination season is due to start shortly for all patients with a chronic disease. To find out if you qualify, please contact us.
  4. DNA's
    In one month alone from August - September 370 patients did not attend their appointments wasting clinicians valuable time. If you cannot make your appointment please contact us in good time.
