In order to register within the Practice, please contact the surgery where you will be asked a series of questions. These questions include, your current address and previous address. The surgery currently has a policcy of only registering patients who are currently living within 'our area' (to find out if you are in our area please call us on 01325 461128). PLEASE NOTE PROOF OF ADDRESS AND IDENTITY IS REQUIRED ON REGISTRATION.

If you are not within our area, you will be advised of a GP surgery nearest to you and contact details for that surgery can be given.

If you are within our area you must call into the surgery and complete a Registration form and a Confidential New Patient Questionnaire, which can be completed by clicking New Patient Questionnaire and Child New Patient Questionnaire for children. We do understand that it is sometimes difficult to recall dates of e.g. vaccinations so please do not worry if you cannot remember, this questionnaire is simply an overview of your medical history to help the Doctors/Nurses appreciate your health needs.

You will also be given our surgeries Patient Leaflet, where any questions regarding the surgery you may have maybe answered.
