In oder to change your address, please call into the surgery and complete a 'Change of Address' form. If you are unable to get into surgery please call us on 01325 461128 and we will be happy to post one out for you.

We encourage patients who move out of our catchment area to re-register with another practice closer to you. This is purely beneficial to the patient in case e.g. a home visit is ever requied in the future.

To change your name, a 'Change of Name' form is required to be completed. We also require proof of change of name which must be photocopied.

To update your contact telephone numbers please contact us on 01325 461128 or call into the surgery where we will be happy to do so.

At times, if we are unable to speak to you via the telephone, we have the facility to send texts, if you would like to consent to us being able to send you text messages, please let us know.

  2. MMR Outbreak!
    Due a recent MMR outbreak, all patients are encouraged to have this vaccination. If you are unsure whether you have had one or not, please contact us.
  3. Flu Vaccinations
    Our flu vaccination season is due to start shortly for all patients with a chronic disease. To find out if you qualify, please contact us.
  4. DNA's
    In one month alone from August - September 370 patients did not attend their appointments wasting clinicians valuable time. If you cannot make your appointment please contact us in good time.
